Congratulations on your FLP Achieve Year 3 Award!​ We are excited to work with you and look forward to a great year ahead.

NEW! We are using Smartsheet to collect and share documents.
Phase 1: We will collect all Contract information via Smartsheet forms. Click here to follow the steps to download forms/templates and links to upload via Smartsheet. Once we have all of your documents, we can process your MOU.

Phase 2: Once your MOU has been processed, we will invite you to a Smartsheet portal for your District. Stay tuned for an email invite to activate your account. The email address(es) you provided during the application process will be the ones to receive notifications.
